Referrals and requests for services are accepted from a variety of sources, including individuals, parents, schools, DCYF, and other providers.

Getting Started
For each service we offer, an initial registration needs to take place. Forms are electronic and can be obtained by calling the office or by having your provider send a referral. Administrative staff will ask about your demographic and insurance information so that we may verify your insurance to confirm that the services you are seeking are covered.
Therapy Clients
Once your insurance is verified and initial paperwork is complete, you will be assigned a therapist or added to our waitlist pending provider availability. Once assigned, your therapist will connect with you directly to schedule an intake appointment and all following appointments will be coordinated with your therapist.
Testing Clients
Once your insurance is verified and initial paperwork is complete, you will receive a call to schedule an intake appointment with a provider tasked to collect the client’s life history to support the need for testing. A testing appointment will be scheduled at the time of the intake appointment.
